Puppy Questionnaire & Information Sheet
This is my "form" questionnaire I send out to anyone considering a puppy from me. Anyone who is in consideration for one of my dogs needs to fill this out so I will have some basic information to help me assess what type dog you would like as well as other pertinent information. The beginning is a bit about me and my objectives, for your information. If you are interested in a puppy, please send back the questionnaire portion with your answers! All information is confidential.
When you return your questionnaire, I will review it and put you on the waiting list. When the pregnancy is confirmed you will be called and at this time I ask for a $500.00 deposit to hold your puppy for you. Final payment is cash-only at pick-up. Personal checks will not be excepted, NO EXCEPTIONS. Show and/or breeding dogs will be $500.00 more than the puppy price with a co-ownership only .Any puppy taken in lieu of a Stud fee will be co-owned as well. There are no exceptions. Thanks!
Reservations for the litters get full very quickly, so the sooner you send it the better. Please let me know what you expect to do with your puppy if you are going to compete in any venue so I have a good idea when I am choosing your puppy. If we are to keep a puppy past pick up date the puppy must be paid in full and a boarding fee will be added to the price of the puppy for food and time.
Thank you for inquiring about my Golden Retrievers. I am a dedicated "fancier" who participates in various dog-related events but primarily in the conformation ring. I breed occasionally to produce what I believe to be quality Golden Retrievers that conform to the breed standard, are intelligent, and have the natural hunting, retrieving, and swimming instincts the breed was bred to possess. I give priority to breeding for health.
Temperament is my top priority and I strive to breed only Goldens that will make wonderful house dogs that are friendly toward all people and other dogs. I also focus on doing all I can to produce physically and mentally sound dogs.
Through competition, membership in the national Golden Retriever Club, articles in magazines, books, and the information shared via Golden and dog-related email lists plus other internet sources, I am exposed to other points of view, educational programs, updated rules and regulations. More importantly, I have an opportunity to share concerns, information and ideas with other concerned and knowledgeable dog people.
When I have litters, I try to make sure all my puppies are sold to loving committed homes. I do my best to screen the owners to ensure a good match between the puppy and the buyer. Before the puppy leaves my home, it has been well cared for and socialized. Picks of puppies are not chosen on a first come first serve basis. I will evaluate each puppy and do my very best to match puppies personalities to those prospective owners. We prefer not to place puppies in families with children under 5 years old unless we feel it is appropriate to do so.
The puppy does not go to its new owner as a trained, well-mannered finished product, of course. It is up to the puppy buyer to complete the job we have started. The new owner is responsible for the further development of the puppy’s personality, through structured discipline, training, guidance, and love. I am available to offer guidance with the care and handling of this pup and both encourage and expect that the new owner will come to me with any questions or concerns.
Before the pups leave here, they are current on their vaccinations and wormed several times.
All of my pet puppies are sold on a Limited Registration, which means that if the dog is bred, the resulting offspring will not be recognized or register-able with the AKC. Pet puppies are required to be spayed/neutered unless at some later date I reevaluate the dog to be of show/breeding quality. At this time, I can reverse the Limited Registration to a full registration. Each individual case is handled as the situation deems appropriate, in my opinion.
As much as possible I attempt to evaluate each puppy’s personality by observing him or her during their development. I also have fellow dog fanciers analyze the litter and follow structured evaluation and temperament tests. I attempt to match the puppy to the buyer to produce the best fit of puppy and family.
Prior to taking the puppy home you will be given information on training and care of your puppy and a list of the proper equipment and supplies to obtain prior to bringing your puppy home. I also will supply you with a "Puppy Packet" which includes pictures of both parents, a copy of the pedigree, a sample copy of our sales agreement, clearance information on the parents, history of the parent's competition in the breed ring, and other information I wish to include.
I make every effort to ensure that my puppies are free from inherited defects, by screening perspective breeding stock for those problems common to the breed as determined by the Golden Retriever Club of America. However due to complex genetic interactions and environmental factors beyond my control, I cannot guarantee that a puppy purchased from us will be free of genetic disorders. The pup is guaranteed to be free of genetic or structural faults that would impact the puppy's quality or length of life. This would include, but is not limited to, conditions such as crippling hip dysplasia or severe heart conditions that would affect the quality and length of this dog's life. It would not include mild problems that may be genetic but will not affect the dog's life as a quality pet.
Please take into consideration the costs of dog ownership and be sure it fits into your budget. You will need to allow for the cost of adequate housing, fencing, quality food, veterinary expenses and health insurance. Also be aware that this breed requires regular brushing and attention.
Please note that we are not a business. You are not clients you are family. Your dogs are my grandpups have no doubt we are dedicated to them. A goldens lifespan ranges from 10-13 years. Longer is amazing and shorter is sad. Problems do arise occasionally if this happens we are here to help. Remember you were chosen.
In an effort to ensure that my puppies are going to receive the love and care they will require throughout their lives, I ask all potential buyers to complete this questionnaire. If you wish to be put on our waiting list you must complete and return the questionnaire to me. If it is a referral from another buyer please put the name of the person that referred you. Please complete the following information and return this to me when done. I would appreciate it if you would cut this first part off before you return the questionnaire.
